COVID-19 Conditions of Entry

Please do not enter the premises or attend your appointment if you have:

returned from overseas or interstate travel in the last 14 days;
knowingly come into contact with someone who has returned from overseas or interstate travel in the last 14 days;
knowingly come into contact with someone who has been tested, diagnosed or exposed to COVID-19 in the last 14 days; or
any potential COVID-19 symptoms including fever, cough, sore throat, runny nose or unexplained shortness of breath.
